※ Personal Information Collection and Usage Agreement
In accordance with Article 5 (Collection and Use of Personal Information) and Article 22 (Agreement Procedure) of the Personal Information Protection Act, the Institute for Basic Science (IBS) would like your consent to the collection of personal information and the use thereof in regard to identifying customers for various services.

Items of personal information collection
All the personal information written in the “Virus request form” (Name, Principal investigator, Department/Address, email, Phone number, etc.)
Period of maintenance and use of collected personal information
1 year (12 months) from the date collected
Refusal right and disadvantages of refusal
The customer has the right to refuse the collection and use of personal information. However, he or she may have the disadvantage of not receiving the service or receiving a notice from virus facility
I fully understand the contents written above, and agree that IBS Virus facility collect and use my personal information in accordance with the related law.
